Office Information: About Your Visit

Insurance Information

We accept most major insurance plans.


Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgeries.

Fees & Payments

We make every effort to decrease the cost of your medical care. Co-payments are due at the time of service. We accept cash, checks, MasterCard and Visa for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.

If you decide to have surgery, you may incur a variety of other charges from providers outside of our office, including: laboratory/blood donation, hospital, anesthesia, physical therapy, home health care, durable medical equipment and/or medications. This is determined by your insurance coverage with your carrier. We do not know the total amount for which you will be responsible until your insurance claim has been processed. However, we can provide you with the surgical CPT code so that you can contact your carrier prior to your surgery.

Prescriptions & Renewals

Please request all prescriptions and authorizations for renewals between 9:00am – 5:00pm, Monday to Thursday, when our full records are available. To expedite the renewal, please request all medication refills from your pharmacy and have them fax the request to 909.629.4704. Renewals requested at other times will be filled only for extreme or emergent circumstances.
